How do Forms work and How to associate it to a Plan?
Student -> Form -> Create Form

Please Note:
Student would fill the form only once. Next time it would not be displayed again to them.
If the same form is associated with multiple Plans, the student would fill it only once.
If different forms are associated with different Plans, the student would have to fill the specific form before they purchase the subscriptions.
